[IUG] Advice On Overdue Mailer Forms?

Can anyone recommend a continuous feed 2 or 3 part carbon mailer form and
printer for overdue notices.   We currently print letters and stuff them in
envelopes.  Any advice on Millennium setup for said method would be
appreciated.
